Add an intake form to your service or product

1. Head over to your Service

2. Edit or Create a service

3. Enable "Customer can purchase this online”
   

4. Click the dropdown and select an intake form from the drop-down list. Customers will need to fill out the form prior to scheduling the service.

   
5. Click on Save button
